• Please register on the Dental Waiting list by leaving your name, address, postcode, date of birth and contact number. Forma are available from Parish Clerk - phone 01748-826445 or email parishclerk@sunshine.gotadsl.co.uk
• Please include full details of each person you want to include on the register.
• Please advise us if you have any disabilities - this will help us allocate you to the most appropriate dental practice.
• Your contact details will be collated and you will be informed by the dental practice when an NHS dental place is available.
